- Signature: A physical or electronic signature of the copyright owner or someone authorized to act on their behalf.
- Identification of Work: Detailed identification of the copyrighted work claimed to be infringed.
- Identification of Infringing Material: Exact location of the infringing material on the Streambia platform. Please include the URL of the page in question.
- Contact Information: Contact information of the complainant, including name, physical address, email address, phone number, and fax number (if available).

- Good Faith Statement: A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Consent to Jurisdiction: Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the courts of the Republic of Namibia, and that you will accept service of process from the person or company who provided the original infringement notification.
